Former San Joaquin County public health leader to strengthen local data exchange and support health organizations in advancing whole-person care

Manifest MedEx (MX), California’s largest nonprofit health data network, today announced Shadi Barfjani, MD, MS, has joined as Executive Director of its Central and Northern Regions. Dr. Barfjani brings more than 15 years of experience in public health leadership, healthcare transformation, population health, and cross-sector collaboration.

Dr. Barfjani most recently served as Senior Deputy Director of San Joaquin County Public Health Services. In that role, she led initiatives to improve community health outcomes, strengthen partnerships, and advance data-driven decision-making.

It starts with investing in and prioritizing seamless data sharing,” said Dr. Barfjani. “In my new role I'm looking forward to helping health organizations better understand the unique needs of different populations and how we can coordinate within all of these different systems to improve community health.”

Before joining San Joaquin County, Dr. Barfjani led the Chief of Programs Division in Plumas County. She helped reduce the impact of vaping on teens and children and served on the Rural Communities Opioid Response Program Planning Grant. She also served as the Vice President of BRAVE Society, a nonprofit focused on policy research and development, and as an assistant physician at Hart Fertility Clinic.

Dr. Barfjani brings deep experience in CalAIM, Enhanced Care Management (ECM), health informatics, and cross-sector partnerships spanning public health, managed care, and provider systems. She has developed health programs in the U.S., Canada, and her native country, Iran, where she earned her medical degree.
