The guide identifies limited HCBS capacity as a recurring problem, with enrollment caps, provider shortages, and long waiting lists restricting access to programs that already exist.
West Virginia ranks last in the AARP Long-Term Services and Supports Scorecard cited in the guide, with Alabama immediately above it. South Carolina, Mississippi, Tennessee, Oklahoma, Louisiana, Florida, Kentucky and North Carolina complete the guide's 10-state ranking, while Texas receives separate attention because of the scale of its HCBS interest lists.
The guide also details how these gaps play out in practice. Disabled adults in Kentucky can spend years waiting for waiver services, North Carolina families may face lengthy delays for the Innovations Waiver, and Texas maintains large interest lists for long-term community support. In rural areas, provider shortages and limited accessible transportation create additional barriers, making services difficult to access even for residents who already qualify.
Diamonds of Disability stresses that the ranking evaluates support systems, not the communities or disabled people who live in them. Some lower-ranked states have comparatively affordable housing, useful individual programs, or stronger service networks in particular cities and counties, so experiences can differ considerably within the same state.
"The goal of naming these gaps plainly is not to discourage. It is to equip," the guide states. Residents are encouraged to join applicable waiver lists as early as possible, look into Medicaid services available outside capped waiver programs, and contact their state's Protection and Advocacy organization or Center for Independent Living for support.
