Payment Changes Require More Than One Budget Assumption
The outlook examines the different payment mechanisms facing hospitals, physicians and post-acute providers rather than treating Medicare's annual rulemaking as one uniform revenue adjustment.
Hospital and skilled-nursing payment rates are increasing. CMS finalized a 2.3% FY 2027 inpatient hospital rate update for hospitals meeting the applicable quality-reporting and electronic-health-record requirements, alongside a 2.4% skilled-nursing-facility rate update. Quality adjustments, case mix, wage indexes and other payment provisions can produce different results for individual organizations.
Physician conversion factors remain on a separate, proposed track. CMS's proposed CY 2027 Physician Fee Schedule conversion factors are $33.17 for qualifying alternative-payment-model participants and $32.84 for other clinicians, representing decreases of 1.19% and 1.68%, respectively, from 2026. These proposed conversion factors are not final rates or uniform percentage changes in every physician's reimbursement.
Black Book's analysis therefore separates facility payments, professional services, diagnostics, pharmacy and post-acute operations. It emphasizes service-line calculations rather than applying a single national update to an organization's entire revenue base.
Funding, Coverage and Drug Pricing Create Distinct Implementation Demands
$50 billion in rural transformation funding. The Rural Health Transformation Program provides $10 billion annually from 2026 through 2030, with awards to all 50 states. The funding supports approved transformation activities; it is not an automatic reimbursement increase or an unrestricted operating subsidy for every rural provider.
Six-month Medicaid renewals for the affected adult population. CMS guidance establishes eligibility redeterminations every six months for most individuals in the Medicaid adult expansion group, beginning with applicable renewals scheduled on or after January 1, 2027. Transition arrangements and eligibility categories matter: the requirement does not apply identically to every Medicaid beneficiary.
Negotiated Medicare prices for 15 additional selected drugs. Prices from the second negotiation cycle take effect January 1, 2027, for the selected Medicare Part D drugs. Negotiated prices and an individual beneficiary's coverage or out-of-pocket obligation are different measures.
These developments sit within a healthcare sector that accounted for $5.3 trillion in spending and 18% of U.S. gross domestic product in 2024, according to CMS. Federal government sponsorship represented 31% of that spending, while state and local governments represented 16%. These are historical measures of public financing's role, not forecasts for 2027.
State Requirements Cannot Be Reduced to a National Checklist
For 2027, California's adopted statewide healthcare spending-growth target is 3.2%, with a separate 1.7% target for identified high-cost hospitals. Massachusetts has maintained its healthcare cost-growth benchmark at 3.6%. These measures have different scopes and accountability mechanisms; they are not interchangeable payment updates or uniform reimbursement cuts.

The rule covers specified Medicare Advantage, Medicaid, Children's Health Insurance Program and federally facilitated exchange arrangements-not every commercial or self-funded employer plan.
