The report arrives as state-level differences in disability assistance become increasingly relevant for people navigating Medicaid, home- and community-based services, and other forms of support under the current administration's financial cuts to disability support systems. According to KFF's 2026 analysis, people with disabilities and adults age 65 and older account for approximately one in five Medicaid enrollees, while more than half of Medicaid spending is associated with these groups because of higher per-person costs. However, Diamonds of Disability explains that states differ substantially in how they structure optional Medicaid eligibility pathways for people with disabilities and older adults.
Access to home-based disability assistance also varies considerably. KFF's 2025 data identified more than 600,000 people waiting for Medicaid home- and community-based services across reporting states, including more than 182,000 seniors and adults with physical disabilities. States use different terminology and eligibility systems for these programs, making it difficult to compare support without examining the underlying policies and services available in each location.
Diamonds of Disability's report evaluates states across eight categories intended to reflect the practical realities of living with a disability: Medicaid waiver access, in-home caregiver support, housing, transportation, vocational rehabilitation, affordability, state tax treatment, and disability-services infrastructure.
Minnesota ranks first, followed by Washington, Massachusetts, Oregon, Vermont, New York, Wisconsin, Maryland, New Jersey, and Colorado. The analysis also identifies tradeoffs within each state, recognizing that strong disability programs may coexist with challenges such as high housing costs, rural provider shortages, transportation limitations, or long waits for particular services.
The report further emphasizes that a favorable state ranking does not necessarily mean every community within that state offers the same level of support. Urban and rural residents can experience very different access to transportation, medical providers, accessible housing, and disability organizations, explains Diamonds of Disability. Individual circumstances also influence which state may be most suitable, including the type of disability, income, employment status, need for personal care, housing requirements, and access to family caregivers.
