According to the wrongful death complaint, Carle suffered severe neglect during her time at the facility, including multiple preventable falls and inadequate supervision. The lawsuit alleged that staff failed to properly monitor residents with dementia and high fall risks, failed to implement appropriate fall prevention measures or provide sufficient assistance with daily living activities, and delayed obtaining emergency medical care as Carle’s health deteriorated.

These tragedies highlight a pattern of severe lapses in resident supervision, safety protocols, and accountability at The Waverly and other long-term care facilities.
