Breast Care Partners, in East Syracuse, New York, recently implemented Oncology OnTrack to support its nurse navigation program. Breast Care Partners was recently founded to ensure delivery of the highest standard of breast care.
PRLog - Mar. 17, 2014 - SYRACUSE, N.Y. -- Breast Care Partners, LLC, (BCP) is a unique partnership between dedicated breast care specialists from CNY Surgical Physicians, St. Joseph’s Physicians Surgical Services, Hematology Oncology Associates of CNY, Crouse Radiology Associates and Crouse Hospital. This partnership was made to streamline the management of malignant and benign breast disease.
Program Director Pat Donnelly and Breast Cancer Nurse Navigators Kathleen Tindall, RN, and Emeline McShane, RN, led the implementation of Oncology OnTrack. They worked with the Priority Consult team to identify the ways that Oncology OnTrack could help them provide quality care more efficiently. The specialized care they provide involves assigning a certified Nurse Navigator to guide each patient through individualized evaluation and course of treatment. Their multidisciplinary team meets weekly to discuss patient cases and develop treatment recommendations.
BCP is pursuing national certification and is also utilizing Oncology OnTrack to help track and analyze de-identified patient data that will be used for quality improvement and benchmarking their program with other centers in the country.
